(1) Fee includes all scientific symposia and satellite symposium, one roundtable, industry-sponsored workshops, 3 lunches, 6 refreshment breaks, Opening Ceremonies and Reception, Exhibitors' Wine & Cheese Reception, transportation to and from Opening Reception and Banquet, President's Reception, Gala Banquet.   It does not include Sunday workshops, or Tuesday/Wednesday Roundtables over and above 1 free roundtable.

(2) Fee includes events of the day, including symposia, industry sponsored workshops, lunch, and morning and afternoon refreshment breaks.  President's Reception, Banquet, and Roundtables not included. Only one day can be attended.

(3) Fee includes Opening Ceremonies & Reception, Exhibitors' Wine & Cheese Reception, transportation to and from Opening Reception and Banquet, President's Reception, Gala Banquet.

(4) All Roundtables are limited to 9 registrants plus the speaker.  Registrants will be accepted on a first-come, first-served basis.  If the program does not accept your choice, it is because the selected roundtable is full.

(5) Golf fee includes green fees plus cart.  Does not include rental fees for clubs.

WORKSHOPS AND ROUNDTABLES:
Registrants will be accepted on a first-come, first-served basis.  Tickets will be issued for all of these events and will be collected at the door.  W1 is an all-day workshop and is offered at the discounted rate of $100 per person.

SPECIAL ACCESS AND FOOD REQUIREMENTS:
Please list essential special food requirements only.  In general, choice of food is the delegate's responsibility.  Special dietary requirements can only be accommodated through advance registration.

CANCELLATION FEE:
Cancellations must be received in writing to the CSCC Conference Office on or before May 28, 2010 in order for registration fees to be refunded.  A service fee of $50.00 will be assessed on all refunds.  No refunds after May 28, 2010.
